Kummell disease (KD) is a rare cause of vertebral fracture due to osteonecrosis. The natural history the characterized by previous minor trauma, subclinical window period, and then symptomatic period presenting with disabling pain, kyphosis, or neurologic deficit. As an important but non-discogenic cauda equina syndrome. Here, we report on wheelchair-bound 28-years-old bodybuilder man KD who presented progressive paresthesia weakness both legs (impending syndrome) L5 involvement. He had past medical arbitrary use licensed unlicensed drugs in fitness field. patient underwent posterior decompression, spinopelvic stabilization, fusion. Postoperative rehabilitation was satisfactory after six months, leg muscle strength returned normal patient's back pain disappeared. should be considered as differential diagnosis when dealing any associated trauma asymptomatic period.
